GOC
CARE Packages have been funded to provide packages of care to
people from Culturally and Linguistically Diverse Background,
in particular to people who identify themselves as being Greek,
have a strong affiliation with the Greek Culture or have a connection
with the Orthodox faith.
What
are Community Care Packages ?
For
many people, living in the comfort of their own home is an important
part of growing older.
Community
Aged Care Packages are planned and managed care services to
help older people with complex care needs remain living in their
homes. They are designed for each individual and are based on
their particular needs.
What help is available?
The
types of services available are specific to each person. They
could include assistance with bathing, meal preparation, laundry,
dressing, transport to appointments, housework and social activities.The
services provided are very flexible and can change as your need
change.
Who
is eligible for Community Aged Care Package?
To
be eligible to receive a Package, your needs must first be assessed
by an Aged Care Assessment Team (ACAT) determine whether you
need the kind of help a Package provides. This assessment is
free.

you live in the Brisbane North Area you will need to get approval
from either:

How
is a Community Care Package arranged?
Your
ACAT assessor will ask you if you have a preference for service
providers. If you elect GOC CARE Packages as your choice, a
copy of your approval will be sent to our offices.
The GOC CARE Packages’ Program Manager will contact you
and your family or representative to arrange a meeting to discuss
your needs and to answer any questions you may have about the
service.
After
you have agreed on your care, a care plan outlining the services
and how they will be delivered will be given to you.
Are there fees for these services?
You
will be asked to contribute financially to your care. If you
are on the maximum basic rate of pension, your fees will not
exceed 17.5 % of that pension.
If
you are experiencing financial hardship, special consideration
will be given. You will not be denied a service you need, because
you cannot pay.
What
are my rights if I receive a Package?
If
you receive a Community Aged Care Package you have the right:

To be involved in deciding which care services are the most
appropriate for your needs;
· To receive services that take account of your lifestyle,
cultural and religious preferences;
· To be able to take part in social activities as you
wish;
· To be treated with dignity and have your privacy respected;
· To complain about the services you receive without
fear of losing services, or being disadvantaged in any way;
and
· To choose a person to speak on your behalf for any
reason.
Before
you start to receive your care, you will be offered an agreement
to sign which clearly sets outs your rights and responsibilities
and those of GOC CARE Packages.
What
if my care needs increase?
Your
care plan will be reviewed regularly to ensure it is appropriate
to your current needs. The Community Aged Care Package is designed
to meet your particular needs, the service you receive will
change as your needs change.
If
your care needs increase beyond what can be provided under a
Community Care Package, you may need to consider other care
options. We will discuss your options with you, and will help
you to find a more appropriate service.
